Are Dental Implants Painful? What You Feel During and After the Procedure

Medically Reviewed 28 August 2026, by Dr. Elian Dylgjeri – Implantologist & Oral Surgeon at Trio Dental Center

One of the most common concerns before getting a dental implant is whether the procedure will hurt. In most cases, implant placement is performed under local anaesthesia, so patients usually feel pressure and movement rather than sharp pain.

After the procedure, some soreness, swelling, and tenderness are normal for a few days. Knowing what to expect before treatment can make the entire experience feel much easier.

Are Dental Implants Painful During the Procedure?

Usually, dental implants are not painful while they are being placed because the treatment area is numbed beforehand. Local anaesthesia blocks pain during the surgical stage, although you may still notice pressure, vibration, or movement.

These sensations can feel unusual, but they should not be painful. If you are considering treatment, you can also learn more about dental implantology at Trio Dental Center and the different implant solutions available.

What Does Dental Implant Surgery Feel Like?

During a straightforward implant procedure, you may be aware that the dentist is working in the area without actually feeling the implant being placed. Some patients describe the sensation as pressure similar to other dental procedures performed under local anaesthesia.

More complex cases involving several implants may take longer, but the same principle applies. The goal is to keep the treatment area numb and comfortable throughout the procedure.

When Does the Discomfort Start?

Most patients begin to notice discomfort after the local anaesthesia starts wearing off. This may happen several hours after the procedure, depending on the anaesthetic used.

The implant area may feel sore, tender, or slightly swollen, especially when eating or touching the gums. These symptoms are usually strongest during the first few days and should gradually improve.

How Much Pain Is Normal After a Dental Implant?

Mild to moderate discomfort is common after dental implant surgery. The level of discomfort depends on the complexity of the treatment and how your body responds to healing.

A single implant may cause relatively little soreness, while several implants combined with extractions or bone grafting may cause more swelling. The important thing is that the discomfort should improve rather than become progressively worse.

Dental Implant Pain Day by Day

First 24 Hours

During the first day, the surgical area may feel tender and slightly swollen. Minor bleeding or sensitivity can also occur while the gums begin healing.

You should avoid disturbing the area and follow the postoperative instructions provided by your dentist. Softer foods are usually recommended during this period.

Days 2–3

Swelling can become more noticeable during the second or third day. The area may still feel tender when chewing or touching the gums.

This can be part of normal healing after implant surgery. Medication recommended by your dentist can usually help control discomfort.

Days 4–7

For many patients, soreness and swelling start to decrease during this stage. Eating and normal daily activities generally become easier as the gums continue to heal.

By the end of the first week, discomfort should usually be significantly reduced. Persistent or worsening pain should be discussed with your dentist.

Is Dental Implant Pain Worse Than a Tooth Extraction?

Not necessarily. Some patients find implant placement easier than they expected, especially when compared with removing a severely damaged or infected tooth.

The amount of discomfort depends more on the complexity of the procedure than on whether it is an extraction or implant. A simple implant can sometimes cause less postoperative soreness than a difficult extraction.

Does Bone Grafting Cause More Pain?

Bone grafting may cause additional swelling and tenderness because more tissue is involved in the procedure. However, the treatment itself is still performed with local anaesthesia.

The amount of discomfort depends on the size and type of graft required. If you have been told you do not have enough bone for implants, read more about bone grafting for dental implants at Trio Dental Center.

What About All-on-4 or All-on-6 Dental Implants?

Patients receiving All-on-4 or All-on-6 dental implants usually undergo a more extensive procedure than someone receiving one implant. Treatment may involve several implants, extractions, gum treatment, or temporary fixed teeth.

Because more areas are treated at once, swelling and soreness can be greater during the first few days. Even so, postoperative discomfort should gradually improve with the recovery plan provided by the dental team.

How Can You Make Recovery More Comfortable?

Following postoperative instructions carefully can help make implant recovery easier. Your dentist may recommend medication, softer foods, good oral hygiene, and avoiding pressure on the surgical area.

You may also be advised to avoid smoking and intense physical activity during the early healing period. Always follow the specific instructions given for your individual treatment.

When Is Pain After a Dental Implant Not Normal?

Some discomfort is expected after implant placement, but pain should gradually become less intense. Pain that suddenly worsens or continues for longer than expected should be evaluated.

Contact your dentist if you notice:

  • Severe or increasing pain
  • Significant swelling that keeps worsening
  • Persistent bleeding
  • Fever
  • Pus or unusual discharge
  • Bad taste together with swelling
  • Implant movement
  • Persistent numbness

These symptoms do not automatically mean the implant has failed. However, they should be checked as soon as possible.

Does the Implant Hurt While It Is Healing?

Once the initial surgical soreness has passed, you should not normally feel constant pain while the implant heals in the jawbone. This healing process is known as osseointegration and usually continues for several weeks or months.

During this period, the implant gradually becomes stable within the bone. If new pain develops after the area had already become comfortable, you should contact your dentist.

Are You Awake During Dental Implant Surgery?

In many cases, yes. Local anaesthesia allows you to remain awake while keeping the treatment area numb.

For anxious patients or more complex procedures, additional options may sometimes be discussed depending on the treatment and medical history. These details should be reviewed with the dental team before surgery.
