Bone grafting before a dental implant: when you need it

A bone graft rebuilds the jawbone where it has become too thin or too low to hold an implant. Many people who lose a tooth never need one; others do, because the bone has shrunk in the months or years since the extraction. Whether you need a graft is not a guess: a 3D CT scan measures the bone before anything is decided. Here is when a graft is needed, the alternatives, how long it takes and what it means if you come to Tirana for treatment.

Why the bone shrinks after a tooth is lost

The bone around a tooth is kept in shape by the root that sits in it and the chewing forces that pass through it. When the tooth is extracted, that part of the bone is no longer loaded and it slowly shrinks, in height and in width. The shrinking is fastest in the first months after the extraction and continues over the years. In the upper back jaw a second factor adds up: the maxillary sinus, the air space above the molars, can extend downwards and leave only a thin floor of bone. How you know if the bone is enough: the 3D CT scan

A panoramic X-ray shows the bone in two dimensions; it cannot show its width. The 3D CT scan (CBCT, €100 at our clinic, included in full-arch packages) shows height, width and density at every point, and where the sinus and the nerve in the lower jaw sit. The implantologist plans each implant on the scan and sees whether it fits in the bone you have.

• Enough bone: the implant is placed without a graft.
• Slightly too thin: a small graft is placed around the implant in the same session.
• Too little bone, or a low sinus: the graft or sinus lift comes first, and the implant follows once the bone has healed.

The kinds of bone graft

• Graft around the implant (guided bone regeneration): graft material is placed where the bone is thin and covered with a membrane that keeps the gum out while new bone forms.
• Sinus lift: in the upper back jaw the sinus membrane is gently lifted and graft material is placed underneath, adding height to the bone floor.
• Block graft: when a large volume is missing, a block of bone is fixed in place; this is the longest route and is used only in larger defects.

The material can be your own bone (taken from the mouth), bone of animal origin processed for medical use, or synthetic material that the body replaces with its own bone over time. Your treatment plan names the material used and why.

Graft and implant in the same session, or in two steps?

When the bone is only slightly too thin, the implant and a small graft can often be placed together, so the graft adds no extra trip. When too much bone is missing, or the sinus sits too low, the graft comes first and the implant follows after the bone has healed, usually several months later. For a patient who travels, the difference matters: a same-session graft keeps the usual two trips (3–5 days, then 4–6 days after 3–6 months); a two-step graft can add a stay. Your written plan says how many trips before you book.

Can a graft be avoided?

Sometimes. With a full arch, All-on-4 places the two back implants at an angle, so they reach denser bone in front of the sinus or above the nerve; that is often chosen precisely to avoid a graft. The CT scan decides whether these options are safe for you; forcing an implant into too little bone to avoid a graft is what makes implants fail.

Does it hurt, and how is the healing?

A graft is done under local anaesthesia, like an implant; conscious sedation (€350) is available if you are anxious. Afterwards expect swelling and soreness for a few days, controlled with ordinary painkillers, and a soft diet on that side. After a sinus lift, avoid blowing your nose hard, and ask the surgeon when you can fly; plan your return date with the clinic before you book. Smoking slows bone healing and raises the risk that the graft does not take.
